In consideration of permission granted by MSHS Wildcat Cheer Boosters allowing my child to participate in activities, events, camps, clinics, practices, fundraisers, or other programs sponsored or supported by MSHS Wildcat Cheer Boosters (collectively, the “Activity”), I represent, covenant, and agree, on behalf of myself, my child, my heirs, assigns, and any other person claiming by, under, or through me, as follows:

1. Acknowledgment and Assumption of Risk

I acknowledge that participating in the Activity involves certain risks and that injuries, death, property damage, or other harm could occur to my child or others. I understand and voluntarily accept all risks of injuries, damages, or harm that may arise during or result from participation in the Activity, regardless of whether caused in whole or in part by the negligence or other fault of MSHS Wildcat Cheer Boosters, Snoqualmie Valley School District, its schools, and/or their respective departments, trustees, directors, officers, employees, volunteers, agents, affiliates, or insurers (collectively, the “Released Parties”).

2. Waiver and Release

I waive all claims against the Released Parties for any injuries, damages, losses, or claims, whether known or unknown, that arise during or result from participation in the Activity, regardless of whether caused in whole or in part by the negligence or other fault of any of the Released Parties. I release and forever discharge the Released Parties from all such claims.

3. Indemnification and Hold Harmless

I agree to indemnify and hold the Released Parties harmless from all losses, liabilities, damages, costs, or expenses, including but not limited to reasonable attorneys’ fees and other litigation costs and expenses, incurred by any of the Released Parties as a result of any claims or suits that I, my child, or anyone claiming by, under, or through me may bring against any of the Released Parties to recover losses, liabilities, costs, damages, or expenses arising during or resulting from participation in the Activity, regardless of whether caused in whole or in part by the negligence or other fault of any of the Released Parties.

4. Emergency Medical Treatment

If I cannot be reached, I authorize the adult leaders supervising the Activity to obtain or authorize emergency medical treatment for my child in the event of an accident, injury, or illness and to act on my behalf in approving reasonably necessary emergency medical care.

5. Photo and Video Release

I understand that photos and/or videos may be taken during MSHS Wildcat Cheer Boosters activities and events. Unless I have contacted MSHS Wildcat Cheer Boosters prior to the Activity to opt out, I agree that photos or videos that include my child may be used or posted by MSHS Wildcat Cheer Boosters on its website, social media accounts, promotional materials, advertising, or other communications.

6. Acknowledgment

I have carefully read and reviewed this Waiver, Release and Hold Harmless Agreement. I understand it fully and voluntarily agree to its terms as the parent or legal guardian of the participating minor.
